New Account Access
To request new account access, the employee's manager must send a written request, on letterhead, which includes their original signature (no initials) to the
Account Representative(s). Each request
should include the following information:

Employee Full Name

Employee EID (Employee Identification Number)

Employee UA NetID

Employee University Email Address
-
Employee Department Name and Number (e.g. Enrollment Management Systems
Group 8107)

Employee Job Title
-
Brief explanation of why access is needed

Manager Name and Job Title

Manager Campus Phone Number
Additional Access for Existing COSMOS Users
